For prospective adoptive families, information pertaining to adoption standards, fees, and policies is listed below as Adoption Information. Families interested in the FLS adoption program are encouraged to review this information prior to submitting a pre-application with Family Life Services.

Prospective adoptive couples are invited, at no cost, to complete a brief Pre-application and Statement of Faith (available below) and return it to Family Life Services. The initial screening process is designed to save couples time, effort, and expense as FLS determines whether it is in everyone's best interest to move forward.

After FLS reviews the pre-application for compatibility and requests more information about the prospective family, a Full Application can be completed by the prospective couple and submitted with a $100 application fee to cover the costs of processing the application with the Adoption Committee (FLS staff and Board). If an application is declined, it may be possible to refer a couple to an agency similar to Family Life Services who might be better able to serve them.
